Those who qualify for Medicaid and other safety-net coverage often can’t use it. Many doctors and dentists won’t take it, and it sometimes doesn’t cover the care they need. Only about 2 percent of Palm Beach County’s pediatric dentists accept Medicaid, for example.
Royal Palm Beach homeowner Randi Solomon said she often goes without her medication for high blood pressure, because her Health Care District Vita Health plan pays just a month toward drugs.
“My husband got hurt on the job, and we ended up having to give up our health insurance in 2006,” Solomon said. “We went from having the Rolls-Royce of health care to practically nothing.”
Her 24-year-old son has health issues, including asthma, but lacks coverage, she said. Their strategy? Hope he doesn’t get sick. Hope they don’t get sicker.
“I had to turn down medication for my asthma the doctor wanted me to use because I don’t have an extra 0 a month,” she said, fighting tears. “What else are you going to do?”
The Tripp Umbach report called on county health leaders to develop a shared vision rather than operating in turf-protecting silos.

The county health department runs six primary care clinics, which serve about 65,000 people a year. Gov. Rick Scott proposed zeroing out those clinics’ state subsidy this year. The subsidy survived the 2011 legislative session, but what’s ahead is unclear.
The Tripp Umbach report also praised the Health Care District’s school nurse program for providing vital care for schoolchildren, and said it should be continued.
Boosting primary care for everyone else will require an investment in doctors and nurses, possibly through better reimbursements and support of residencies, the report said.
Palm Beach Gardens pulmonologist Joseph Giaimo said most young doctors won’t consider primary care because they can’t hope to earn enough to pay off their medical school debt, which now averages almost 0,000 per graduate. Better reimbursements, proposed in the federal health overhaul, will help.
“The reimbursement for primary care has not been very good, so they choose the sub-specialties,” he said.
He’s helping develop an internal medicine residency program for Palm Beach Gardens Medical Center. Training programs are costly to develop and need support, he said.
“Florida has a number of medical schools but not a lot of training programs,” he said. “When doctors decide where to establish a practice, they’re most likely to settle where they do their residency.”
The Palm Beach County Health Care District, the Quantum Foundation and other foundations paid for the Tripp Umbach study.
Quantum Foundation President Kerry Diaz said her takeaway is that the county’s leaders need to agree on taking the next step: Meet jointly and focus on a shared strategy for using limited health care resources, both from the foundations and government agencies, in a way most likely to boost the health of county residents.
“It would be my hope that we can all come together and agree that it’s a priority that makes sense for this community, that it’s a wise investment,” she said.
